Only changes in file system clusters are copied when creating incremental/differential images. So, if the incremental image was 20GB this is how much data has changed in source file system including VHDX files.
The problem is that the VHDX file contains pagefile.sys and other system/snapshot files such as restore points that as soon as you boot the VM may result in many GB of changes to the VHDX. If you require granularity to exclude pagefile.sys, hiberfil.sys and restore points then you'll need to run Macrium Reflect from within the VM.
